The finding is explored further: what supports it, what contradicts it, and what is still missing.
The decision is recorded, along with who made it and why.
The next step is prepared and assigned for a person to review or approve.
What happened next is recorded against the original finding and decision. This last step is crucial - most AI tools stop at recommendation. Operations closes the loop.

What it looks like
Follow a decision all the way through
A useful finding is only the start. Operations keeps what happens next connected to the original evidence, so you can see what was decided, what action was taken and what happened as a result.
Finding
A question is asked or something important is identified. In this example, member retention in the London region has fallen below the national average, with the biggest drop among Associate members of three to five years’ standing.
Decision
The team reviews the finding and agrees to test a targeted renewal intervention for the affected members. The decision and the reasoning behind it are recorded.
Action
The agreed activity is assigned, with a clear owner and status, while the original finding and supporting evidence remain attached.
Outcome
When the results are reviewed, the outcome is added to the same record. The organisation can see what was found, what it decided to do and whether the intervention made a difference.
